      Looks like you have been a victim of automatic "Google Slapping". http://www.webmasterworld.com/google_adwords/4020049.htm

      Have all Keyword quality scores got slapped to "1 out of 10" - you can check by clicking sorting ascending by QS in Adwords Editor?

      Were you running any white label affiliate campaigns? (i.e revenue share with another affiliate, and was the other affiliate also banned on 7th of November?)

      As a work-around you could export the account using AdwordsEditor to a new Adwords account - then ask Google account manager to link the CTR history in the back office. But make sure you have removed the dodgy campaign or advert first.

      Two other possible causes:
      * check robot.txt to make sure the ppc landing page bot has not been accidently been blocked (User-agent: Mediapartners-Google*)

      * Were you running any "Make money on Adwords" campaigns?

      The Adwords Ads not running mystery has been resolved.  Thanks to everybody for the detailed troubleshooting tips.  It was a great learning experience.

        The reason for the cancellation was billing - : credit card was declined once (expiration date change) in a 7+ yr period and alternate number was given.
         
        Looks like after all A has been a victim of the Google Slap, but not for the usual Landing Page Quality Score (ranges between 7-10), but for a simple reason that may have happened to any one of us. 
         
        Yes, A was looking into alternate options for their search engine needs and this disappointing experience with Google makes it even more necessary.
