      Updated Group C results from testing SOAP 1.2 endpoints:

      The implementation here has been modified so that when header blocks are
      received without an encodingStyle specified, an attempt is made to find a
      matching soap:header(fault) binding element in the WSDL doc so that the
      encoding style can be determined. If no match is found, it's assumed to be
      a "literal" block. This way encoded blocks without the optional
      encodingStyle attribute can be detected as long as they are defined in the
      WSDL.

      Robert -- this fixes the issue with gSOAP response headers but unmasks a
      problem with the "struct" header... the client marks those operations as
      fail because of a loss of precision in the float member of the echoed
      struct.
