It would not be inappropriate to use Miko in front of her name as an "unofficial" title (unofficial under SCA terms). Though like any other title, official or not, she would not include it as part of her name when submitting it for registration. For example, you would not include Lord/Lady when submitting a name for registration, etc.
refer to someone as "Miko-sama", but I don't know that you would refer
to them as, say "Fujiwara Miko-sama" (though that would fit the
general rules). I'll check with our Japanese members, though.
