I sees this in the log:

(II) Module nv: vendor="The XFree86 Project"
      compiled for 4.1.0, module version = 1.0.1
      Module class: XFree86 Video Driver
ABI class: XFree86 Video Driver, version 0.4

      but this in the config:

      #Driver "nv"
      Driver "nvidia"

According to the log, you're loading the 'nv' driver that comes with X,
not Nvidia's driver. My log has:
      (II) Module nvidia: vendor="NVIDIA Corporation"
      compiled for 4.0.2, module version = 1.0.2802
      Module class: XFree86 Video Driver

I'm not sure why your config didn't load the correct driver. Did you not
try it again after the last config save?
