      You need to decide if the total is US or World wide (include or excluding prototypes) (include or exclude specials or limited releases) (include the various repair and diagnoistic carts); if USA:

      How about:

      5200 Meteorites

      7800 Sentinel is supposed to be the rarest but seems to be only released as PAL. NTSC (US in prototype) I'm not entirely sure which of the last batch got a USA release and which were Foreign only... If USA Maybe something like Mean 18 Ultimate Golf, otherwise Tank Command.

      2600- Magicard (which enabled you to program your own games) was released with only 500 being made available.  It seems higly unlikley that any other vintage game was produced in such a small quantity... however since programmers & collectors were the most likely to buy one a good percentage of these still exist in collectors hands... (too limited? an item) Maybe Eli's ladder? Tooth Protectors?

      NES- Tetris by Tengen (pulled from market) (although copies are available) Nintendo also made a special cart for one of their contest with Mario and two other games on it, which the contestants played in the contest (the games aren't rare but that 3 in one cart is)

      Vectrex- Polar Rescue (released as they were pulling the plug on the system)

      Channel F- Casino Royale may or may not have been actually released.  If released and not just a prototype.... (Or Demo Cart 1 and 2 these were released to the stores for demostration purposes and thus had a limited release... but not to the public) eleminate these two maybe Slot Machine..

      RCA Studio II the only one I'm missing is: Gunfighter/Moonship Battle so I'll nominate that

      Sega Master System- Buster Douglas Boxing is probably the rarest US release.  But some of the last US releases came in the same packages as the International version, If you ignore these Ghouls and Ghost may be the rarest item sold in the USA with USA /not international packaging. And some stuff ...Danian the Jungle Fighter, Scambled Sprits, was unofficially sold in the USA although released in Canada, but these were the regular international versions too.

        One of the first games that I had when I got my original Atari 2600 (when I was like 12) was Tooth Protectors (my parents were real good at picking 'cool' games.)  The other was some program that went along with the lightpen, basically so you could just draw on the screen.  I never saw another cart that used the lightpen after that....
         
        I wish I still had those two things....

                 There was a light pen for the Atari 400/800 computer, and so the end din plug would also fit a 2600.  So many things were proposed for the 2600 and never made so it is likely that such was proposed.but I was unaware of it ever having been made.  The Tooth Protectors I was speaking of has a "Johnson & Johnson" (the bandaid people label)  Do you know anything about it? How it was issued?  I had assumed it was a mail in type offer, like Chase the Chuckwagon and Kool Aide Man.
                There were so many  items announced that never seem to have been produced... I'd have loved the arcade coke machine. You bought your coke and got to play a little videogame within the machine.   I seem to recall that someone was making a game for dentist, the kids would play that in the waiting room, while waiting to get their teeth cleaned.  If this was a promotional item J&J used for advertising to dentist, this would help to explain why the game is so rare, and they certainly could have used a light pen if they wanted too. (the trick would be programming the cart.)

The other was some program that went along with the lightpen, basically so you could just draw on the screen. I never saw another cart that used the lightpen after that....

                  Is it possible I might be confusing the lightpen / Tooth Protectors on the Atari with the Commodore 64?  I got the Commodore 64 a few years after my Atari, so it's possible I'm confusing the two systems....

                       There was a light pen for the Atari 400/800 computer, and so the end din plug would also fit a 2600.  So many things were proposed for the 2600 and never made so it is likely that such was proposed.but I was unaware of it ever having been made.  The Tooth Protectors I was speaking of has a "Johnson & Johnson" (the bandaid people label)  Do you know anything about it? How it was issued?  I had assumed it was a mail in type offer, like Chase the Chuckwagon and Kool Aide Man.
                      There were so many  items announced that never seem to have been produced... I'd have loved the arcade coke machine. You bought your coke and got to play a little videogame within the machine.   I seem to recall that someone was making a game for dentist, the kids would play that in the waiting room, while waiting to get their teeth cleaned.  If this was a promotional item J&J used for advertising to dentist, this would help to explain why the game is so rare, and they certainly could have used a light pen if they wanted too. (the trick would be programming the cart.)
