      Joe,
      I have a copy of "The Army in the Civil War Volume XIII, Statistical
      Records" by Frederick Phisterer - Charles Scribner's & Sons Publishers, 1882
      that lists in detail and catorigorizes the causes of deaths by disease. It
      does not however, give places where these deaths occured nor names. It lists
      141,155 White Troops and 23,408 Colored Troops for a total of 164,563. These
      are deaths in the Union Army and it gives no record on any Confederate dead
      from disease or any other statistics on Confederate losses.
