    Community Health Program Rep

    There are two 25% time peer counselor positions open – one is with
    Community Focus Case Management with an emphasis on Dual Diagnosis;
    other is with Citywide Case Management.

    Community Focus and Citywide are both UCSF agencies, under the
    of Psychiatry at SF General, that provide intensive case management to
    adults with severe mental illness, who often have co-existing
    abuse and medical issues. The programs are co-located and offer some
    overlapping services such as groups and a vocational program. The
    incumbents will work with clients from all programs.

    982 Mission St., San Francisco
    The position involves time at the Center, and in the community.

    · Provide support to clients with severe mental health and dual
    diagnosis disorders
    · Provide resource information, accompaniment, and advocacy
    · Participate in team and program activities, working
    with case managers
    · Serve as a role model of recovery
    · Co-facilitate groups; participate in Center Task Force Meetings

    · Graduation from High School or a GED plus one year experience in
    health or social service position
    · One year of verifiable experience working with a culturally
    · Ability to problem solve, and to prioritize among multiple
    · Ability to work independently, and to work as part of a team
    · Ability to maintain records and charting on the computer
    · Personal experience in the mental health and/or substance abuse
    system as a consumer.
    · Knowledge of resources, advocacy and case management skills
