      Community Case Managers (San Rafael)

      Case Managers: Non-profit social service agency seeks qualified Case Managers to conduct screening and assessments, make referrals for placement in substance abuse treatment, and provide follow-up services, including coordinating community services, developing employment and housing resources, maintaining participant files and documentation of services, collecting and analyzing participant data, conducting visits with treatment providers, and engaging in ongoing communication with program participants, treatment staff, and parole agents of record. Positions are available throughout the San Francisco Bay Area. The requirements for the position include, but are not limited to the following:
       Must have a BA/MA in a relevant field (counseling, psychology, sociology, etc.) OR
       Substance Abuse Counseling Certification from a State of California Accredited Institution;
       Experience addressing substance abuse and criminal justice populations (state parole and probation) is necessary;
       Experience with substance abuse treatment models and an understanding of treatment modalities is expected;
       Knowledge of community resources and services is preferred;
       Must be able to obtain a California Department of Corrections security clearance;
       Must possess a valid driver's license and have a clean driving record.
