FormMail.pl - please help

      I downloaded and installed FormMail.pl (http://www.scriptarchive.com/download.cgi?s=formmail)

      Can someone please tell me what I need to change in the FormMail.pl file to get my form to work on my website?  I put the FormMail.pl file into my cgi-bin directory.  Here is the (top part) of my HTML Code in my form. 

      <form action="/cgi-bin/FormMail.pl" method = "POST">
      <input type="hidden" name="recipient" value="wuv2skate@...">
      <INPUT TYPE="hidden" NAME="subject" VALUE="Response Form">

      It just gives me the error message below.
