Mike McCann's website contains the info you are seeking, but it approaches
the information in a reverse way from the question you are asking. When you
go to the website, you first choose a particular minor league. When you
click on that league, you'll come to a table showing all the teams in that
minor league over the years of its history. As you scroll to the year in
which you are interested, you will see the names of the teams in the league
along with their major league affiliations. The URL for McCann's website is:

      http://www.geocities.com/big_bunko/total.htm

      Hope this helps.
