McMaster-Carr sells lantern style tool posts and rockers, but I think they
may be slightly larger than what you specify.
You might find it easiest to make one. But that might be tough to do without
a working lathe....
You'll find that most people prefer a tool block or quick change setup for
its greater rigidity.
