Well, I think all the implementors would agree that the Candidate
Recommendation is not really good enough. On the other hand, I for one
have actually worked on improving it much, so I cannot really throw
stones at the WG.

the (updated) FO chapter of Harold's XML Bible is good.
otherwise, study the FOP and RenderX example files.
