      Those aren't errors in any fashion ... the book documents that all
      examples
      are using the default namespace for the XSL-FO vocabulary. If you are
      using the fo: namespace for your XSL-FO, then you'll have to add
      prefixes
      .. I don't ... I work in the default namespace for all my XSL-FO work.

      Prefixes are *not* normative ... there is *no* obligation to use fo: in
      your documents, unless you've declared that to be your prefix for the
      vocabulary. I think they clutter the result to the human reader ...
      certainly the computer doesn't care which of the prefixes you use.

      I hope this helps clarify the correctness of the examples in my books.
