      Your sample works fine in XSL Formatter, assuming a font size of 12pt
      (you didn't say what the font size is).

      Could be a bug in FOP's table processing. You might try setting
      table-layout="auto" and see what happens. This should be fine since you
      have explicit widths on all columns.
