      XSL-FO 1.0 allows only equal width equally-spaced columns in a
      <region-body> of a <simple-page-master> using:

      column-count="" column-gap=""

      Block-level constructs can either live in one column or span all columns
      (not a subset of columns) by using:

      span=

      Spanning all columns will cause previously flowed blocks in the page to be
      rebalanced and reflowed in all columns before the spanned construct.
