      Or alternatively, if the subject matter really is like the example above, you could experiment with list constructions: whereby the page number on the left is a 'list-item-label', the text entry and leader and reference number on the right are all within a 'list-item-body', and the pair are contained within a 'list-item', which may have siblings, and they all live happily within a 'list-block'.
