        <svg:image width="200" height="200" x="0" y="100"
        xlink:href="data:image/png;base64,iVBORw0..."/>
        </svg:svg>
        </fo:instream-foreign-object>

        Make sure you have enough height and width. And, make sure you have
        compatible jar files (ie. fop, batik from the same fop package).

        use this example for jpegs (change png to jpeg) too.
