      I agree with Linda that there should be a mechanism so that your software automatically inserts this information onto the claim directly from your patient account. We register this information into our system upon the patients first visit and completion of the initial paperwork. Then we update it as needed only.
       
      You may want to check with your software vendor to see if this can be fixed.

      The information about relationship and any secondary information should be
      contained in the "patient" data and unless changed should transmit the same
      info when sending claims.

      Indeed these are mandatory fields on the CMS 1500 form.

      I personally have not seen a software where this info is keyed in claim by
      claim but rather included in the patient demographics. Also this information
      should be on the patient Info sheet your office has the patient complete
      upon first visit.

      I'm new to the billing field, so please excuse me if I'm asking the
      obvious.

      Our software does not allow us to process claims without providing
      information to the insurance company that it already has. Specifically,
      I'm referring to Relationship to the Insured and Insured's birth date.
      It's even more annoying, because these fields do not seem to be required
      on the 1500 form.

      I have two questions:

      Does all medical office software require this information?

      What do you do when the information is not available?
