How come there was no guarantor/parent signature at the time of service (as New Patient)? Why don t you try speaking with the parent who is the policy holder?

Why don't you try speaking with the parent who is the policy holder? afterall, you billed the insurance (and in my understanding the parent is on the claim as a policy holder).

But the question is whom to bill---- I would say the policy holder who is the parent.

          Good luck!
