      I have some for you:

      - The Government of Ontario (Canada) who's "Drive Clean Programme" issues
      notices in MM/DD/YYYY format for vehicle emissions tests. Note that the
      Driver's Licence uses the correct YYYY-MM-DD format. You can tell them off
      at www.driveclean.com

      - The Toronto-Dominion Bank whose forms use a mish mash of MM/DD/YYYY,
      DD/MM/YYYY and YYYY-MM-DD. They can be reaced at www.td.com

      - Siemens at www.siemens.com who make various products but use date formats
      DD.MM.YYYY, MM/DD/YYYY and rarely YYYY-MM-DD.
