The Disease Management Forum is an
Internet discussion venue open to anyone
involved professionally in disease
management. The goal is open sharing of
information about what disease
management programs are being
implemented, what is working, and what
is not.

Physicians, medical directors, managed
care organization executives,
pharmacists, nurses, students and any
others professionally involved in
disease management.

Diseases for which effective programs
can be created; guidelines that may be
used; methods of measuring outcomes;
available software, and more.


How does it work?

The Disease Management Forum is a
