Per Encoder Pro – there is an NCCI conflict with these two codes but a modifier is allowed – bill 13151 first as it has higher RVUs then 10121-59. From:

There needs to be direct supervision if billing to Medicare and payers that follow Medicare. The administration is billable. A note must be written about the

I don't have an answer for you on that except to say that 302.0 really would likely have been incorrectly used for that purpose as I believe F66 would.
