I'm finding this a little non-intuitive to navigate. Here are some suggestions:
In the "Options for this panel" Dialog, all existing templates should show up under the "Metadata Templates" item
There should be a "Make New Template" item in that list, which takes you to the Extras panel
In the Extras Panel.
I find the "Make Empty template" button confusing to use. There's no clue as to what to do once that template has been made. I needed to try several times to make one, even after reading the user guide. And the only way I could tell I made one successfully was to open the template in a text editor.
I suggest adding other buttons as outlined below.
Add other buttons in the box with "Make empty template" One should be indicate "Make Template from Creator Info" This would create a template that is populated with all the info from the Creator panel. Ideally, this would pop up a dialog that shows all the information, and have a place to name the template. (BTW, I would also add the Credit Line, Source, and Copyright notice from the "Status" panel to Creator. It might also include the PLUS ID of the photographer/rightsholder)
Another button would be named "Make template with all info". This would also pop a dialog showing the values that would be written to the file, and include a box to name the template.
I think there should be an "Inspect" button, along with the Rename and Delete buttons.
So we would have three "Create" buttons - Empty, Creator, and All. And we would have three function buttons - Inspect, Rename , and Delete.

Making this dialog work for both the creation of the templates, as well as the application of the templates leads to confusion for me. Here's one way to help sort it out.
Add a new top line with two options with radio buttons. "Apply Template" and "Make Template"
Left side stays the same
On the Right side, put radio buttons next to "Append" and Replace"
Add an "Inspect" button that would pop a dialog showing all included values.
On the bottom line, include two buttons - "Apply" and "Cancel"
Alternately, you could have two different dialogs - one for creation of templates, and one for application of a template.
