Have you taken a look at how long the hammer's sweeper tentacles are at night? Check to see that they can't reach the toadstool. 3" may be a little too close....

Rock or sand anemone, it's not generally hosted by clowns, but any thing can happen in a tank. I have a clarke that hosted one for a while....

Calcium reactors add tons of calcium in a usable form to your reef. They are low maintenance in that you aren't adding calcium supplement twice a week or kalk...

Check your water parameters; especially ammonia and nitrites. Also ensure that he is getting a diet full of meaty foods (frozen brine shrimp ect.) No idea why...
