Study Finds Physicians Recommend Different Treatment Decisions For Patients
Than They Choose For Themselves

CHICAGO-The act of making a recommendation appears to change the way
physicians think regarding medical choices, and they often make different
choices for themselves than what they recommend to patients, according to a
survey study published in the April 11 issue of Archives of Internal
Medicine, one of the JAMA/Archives journals.

"Patients facing difficult decisions often ask physicians for
recommendations," the authors write as background information in the study.
"However, little is known regarding the ways that physicians' decisions are
influenced by the act of making a recommendation."

Peter A. Ubel, M.D., of Duke University, Durham, N.C., and colleagues tested
whether making a treatment recommendation changes the way physicians think
about medical decisions. The authors surveyed two samples of U.S. primary
care physicians, and presented each with one of two clinical scenarios.

In the first scenario, 500 physicians were asked to imagine that either they
or one of their patients had just received a diagnosis of colon cancer and
faced a choice of one of two operations to treat the cancer. Both surgeries
cured the colon cancer in 80 percent of patients, however one surgery had a
higher mortality (death) rate, but fewer adverse effects, whereas the second
surgery had a lower death rate but a small percentage of patients
experienced colostomy, chronic diarrhea, intermittent bowel obstruction or a
wound infection.

A total of 242 physicians returned the colon cancer questionnaire (response
rate of 48.4 percent), and when asked to imagine they had received the
cancer diagnosis, 37.8 percent of physicians chose the surgical procedure
with a higher rate of death, but a lower rate of adverse effects.
Conversely, when asked to make a recommendation for a patient, only 24.5
percent of physicians chose this option.

The second scenario asked 1,600 physicians to imagine that a new strain of
avian influenza had just arrived in the U.S. One group of physicians were
asked to imagine they had been infected, and the other group was asked to
imagine that his or her patient was infected. One treatment was available
for this strain of influenza: an immunoglobulin treatment, without which
persons who contract flu have a 10 percent death rate and a 30 percent
hospitalization rate with an average stay of one week. The treatment would
reduce the rate of adverse events by half, however it also causes death in 1
percent of patients and permanent neurological paralysis in 4 percent of
patients.

The avian influenza scenario was returned by 698 patients (response rate of
43.6 percent), and 62.9 percent of physicians chose to forgo immunoglobulin
treatment when imagining they had been infected, to avoid its adverse
effects. However, when imagining that a patient had been infected, only 48.5
percent of physicians recommended not getting the treatment.

"In conclusion, when physicians make treatment recommendations, they think
differently than when making decisions for themselves," the authors
conclude. "In some circumstances, making recommendations could reduce the
quality of medical decisions. In at least some circumstances, however, such
as when emotions interfere with optimal decision making, this change in
thinking could lead to more optimal decisions. In debating when it is
appropriate for physicians to make treatment recommendations to their
patients, we must now recognize that the very act of making a recommendation
changes the way physicians weigh medical alternatives."

1. Sacks of Grass Seeds (any variety like wheat, spelt, rye, etc)
2. Reservoir of Water
3. Bags of Salt

Directions:

1. Make pasta: grind up grass seed into flour, add water, flatten, cut into
strips, and dry.

2. Make Sauce:  add egg yolks to dairy

	 To make egg yolks: feed grass seeds and water to chickens and take the eggs

	 To make dairy: add grass seeds and water to grazing land for cows. Milk the
cows. Decant the fat for cream, and rot the rest for cheese

3. Add sauce to pasta: Boil the pasta and fry the sauce, then toss

4. Add salt to Taste: add whatever other seasonings you like, since cooking
kills the taste too

There seems to be some correspondence between man`s evolution over the ages and
his development in the womb. It was first proposed by Ernst Haeckel about a
century ago. By studying ontogeny (the development of embryos), scientists can
learn about phylogeny (the evolutionary history of organisms.) Embryology and
archeology have both contributed important pieces to the puzzle of our ancestry.
Today, we have other tools like genetics. For example: the genes from man have
matched up with our closest relative, the chimp. We are over 98 percent
identical to chimps, making man closer to a chimp than to a girl ... which
really explains a lot.

As a young student at Emory University Medical School, armed with the Atlas of
Human Embryos and the 29th edition of Gray's Anatomy, important nuggets of lore
wafted my way. For example: in the fourth to seventh week of human development,
we are heavy into our reptile phase, with a cloaca. 250 million years ago we
didn't have a urorectal septum to separate stuff. (Yeech!)

But the reason I recapitulate now, is to share my latest epiphany with you. By
tracing backward man's development, all the way to the beginning, when there was
just one cell on earth, the environment of the earth three billion years ago can
be elucidated. At the first moment of life on earth, when the soup du jour was
primordial, we can be sure about what the earth was like at that very first
moment:

Even though there were no fish yet, the oceans already started to smell like it
… and the whole earth was pink … and I also think there was a lot of hair.

* Women who start their menstruation early and/or go into early menopause, increasing the length of reproductive years, are at greater risk.
* Obesity. Women who are overweight are at increased risk
* Women who have never had children are at greater risk.

* Previous breast cancer. * Previous endometrial cancer.

Aside from the genetic predisposition, the common factor in many of these risks is increased endogenous estrogen exposure over a long time.

The resulting disease is rare form of cancer in which malignant (cancerous) cells are found in the mesothelium, a protective sac that covers most of the body's internal organs.

Mesothelioma occurs more often in men than in women and risk increases with age, but this disease can appear in either men or women at any age. About 2,000 new cases of mesothelioma are diagnosed in the United States each year. Although reported incidence rates have increased in the past 20 years, mesothelioma is still a relatively rare cancer.

There are various procedures used for the treatment of mesothelioma. The type of treatment depends on the location of the cancer, the stage of the disease, and the patient's age and general health.

A common treatment of the disease is by means of surgery by the removal of part of the lining of the chest or abdomen and some of the tissue around it. For cancer of the pleura, a lung may be removed in an operation called a pneumonectomy. Sometimes part of the diaphragm, the muscle below the lungs that helps with breathing, is also removed.

Another method is Radiation therapy, also called radiotherapy. This involves the use of high-energy rays to kill cancer cells and shrink tumors. * Obesity. Women who are overweight are at increased risk
* Women who have never had children are at greater risk.
* Women who had their first child over age 30 are at greater risk.
* Previous breast cancer. * Previous endometrial cancer.

Aside from the genetic predisposition, the common factor in many of these risks is increased endogenous estrogen exposure over a long time.
