Hello,
It is me again requesting advice for a young PT entrepreneur. 
His big question is how can he become a provider to various insurance companies?  Where does he get phone numbers and names of insurance companies?  He has looked into PTPN but he questions the value considering the big money initial investment.  And also does he need to become a provider of Medicare and have a Medicare # in order to become a provider with other insurances.
If you have any answers from your years of experience then please respond and I will forward the info to him.  He has been most appreciative of the information he as received thus far.

the space I will be renting is in a fitness facility
but is only big enough for about two
tables(10X10sq.ft.)were I plan to do my hands on and
modality work, then use the fitness facility for the
ther ex. My question to those that might know... Do
clinics of this size with one practioner normally get
accepted into Medicare? I quess the age old question
still applies... Does size matter? Thanks again for

individual number.  To my knowledge, you can
apply for a part B number like a physican as long as
you have an office.  I think... and I could be
wrong (it has happened once or twice) getting a part B
number is far easier than getting an A (even though
when u get an A it bills B). The other difference also
is that part B numbers are capped at $900. 
However, I hear and correct me if anyone knows for
sure, you have an easier time getting reimbursed under
B</P>
<P>As far as PTPN, it is very expensive, and I dont
recommed it to a small clinic, we investigated it, and
there are other insurance networks far cheaper. Their
used to be a list of them in an online artice on
Advance, as well as an artice (also advance but in the
mag, bout 1999)  about how helpful it was to
therapists to be in these networks.  I live in a
small rural area, and financially it didnt make sence,
due to most of the insurances no-one carried. 
The bulk of our business is Medicare, however whenever
we do get private (jackpot) we do get reimbursed, only
a few times did it happen that they wouldnt pay
because we werent a provider.</P>
<P>So what you do is call and verify insurance. 
If they dont take you then ask for an application,
process takes 3-6 months, but it does help to have the
medicare number.</P>

A medicare number is vital in all he does.  Almost every provider and
network requires submission of the number.

I am located in Miami, Florida and set up a small clinic 5 months ago inside a
local gym.  I essentially operate as a business within a business.  The best
thing to do is to get an individual medicare number and keep it simple
especially since you are a small practice.  (My clinic has only one treatment
table).  The medicare packet is not complicated, but you do need to have a few
things in place.  One of the most important things you need is an occupational
license.  In order to get one, you must have a certificate of occupancy.  In
addition, it is important to know that medicare will not allow your business to
be located out of your home.  I have been exploring possibilites and following
all of this stuff for about 6 months now.  It is one of the reasons I started
my clinic.  Next, there is no cap at the moment, at least in Florida.  Medicare
does have a different fee schedule for differnet states.  I have recently
compared Florida's fees with a friend of mine who is starting a business in
North Carolina.  I will receive more for the same services than he will, but
the differences are not severe.  It is also my understanding that the cap was
repealed last year.  I believe it will be up for discussion and a vote sometime
next year.  I am not certain, but I would contact the APTA for an amswer.  The
had a lot to do with the lobby to get the cap repealed.  In Florida, it was

To clarify, if you have an Medicare A number, the cap was $1500, which has been lifted for the next year or 2 until they can figure out what to do with it.  If you have a Med B number, you do have a cap of $900, but have an easier time with reimbursement.  They may have changed this recently, but I have not heard anything about it, and the therapists that I do know with a B numbers claim it still exists. I could be wrong, I have an A number, Ill do some more investigating...

Out of curiosity, did you need to get your building surveyed when appling 4 and individual number, and how long did it take for you to get it??? Thanks Kerry

In the best interest of brain-injured children, a new listserv has been formed
to help parents and advocates access 3rd-party reimbursement of Hyperbaric
Oxygen Therapy (HBOT).

New subscribers will automatically receive a pdf file explaining how the US
federal Medicaid law for children specifically applies to HBOT. New
subscribers also receive a pdf of the Georgia court decision overturning
Georgia Medicaid's denial of HBOT.

Since federal law is applicable in all 50 US states, this court decision
could be used as a model for other states. In addition, many countries
around the world base their healthcare decisions on what happens in the
United States.

Since this listserv began on September 21, 2001, five states have indicated
they will change their policy decisions on Hyperbaric Oxygen Therapy for
brain-injured children: Alaska, Arkansas, California, Texas, and Florida.

Additionally, Virginia, West Virginia, North Carolina, South Carolina, and
Missouri have reimbursed Hyperbaric Oxygen Therapy for brain-injured
children. Let's keep this list growing.

The off-label use of Hyperbaric oxygen has been proven to improve recovery
in brain-injury. There are reports that over 90% of all brain-injured
children who receive HBOT receive improvement. However, it is currently not
accepted by pediatric neurology as a standard treatment of care for our
children. Because of this nonacceptance parents are forced to pay for
hyperbaric oxygen out of pocket. Many parents have already taken out 2nd
and 3rd mortgages to pay for this incredibly safe, noninvasive,
life-changing, permanent therapy.

The goal of the MedicaidforHBOT listserv is to change this status. Once
Medicaid reimburses HBOT and universal improvements occur, the insurance
companies will quickly follow suit. At the same time, this will force
accountability onto Pediatric Neurology, as pediatric neurologists will
have to publicly explain how 90+% of every brain-injured child can improve
from HBOT when most have said such improvement is impossible.

The practice of pediatric neurology is primarily the administration of
palliative care. This branch of medicine is predicated on the obsolete
belief that brain-injury is irreversible and that nothing can be done to
recover lost function.

The vast majority of patients in pediatric neurology are children who
suffer from some sort of seizure disorder. Consequently they are prescribed
anti-convulsants. 100% of the anti-convulsants prescribed for brain-injured
children by pediatric neurologists have never been subjected to randomized,
double-blind controlled studies where children were the subjects in the
study--which means these drugs are not FDA approved and are prescribed
off-label for pediatric use.

As parents we are asking for this same standard--the off-label use of HBOT,
a treatment modality that has been proven to improve recovery from
brain-injury. HBOT permanently and safely *repairs* the source of the
problem where typically prescribed treatments (Speech Therapy, Occupational
Therapy, and Physical Therapy) might briefly alleviate symptoms but never
address or repair the core problem.

Unfortunately, at this time, the Pediatric Neurology community typically
reacts emotionally rather than rationally to the science supporting
hyperbaric oxygen for brain-injury. Most will not even discuss the issue.
Because they hold credentials of perceived expertise, these palliative-care
physicians have been allowed to dismiss HBOT out of hand.

This is frustrating for care administrators and parents as we all seek to
help our children. It is hoped and anticipated that the MedicaidforHBOT
forum will help bridge this gap.

Knowledge and informed decision-making can avoid the adversarial stance
taken in Georgia and truly help these children maximize their full
potential. Your list membership guarantees free access to a library

pdf files relating to the efficacy of Hyperbaric Oxygen Therapy. Take
advantage of it.

Since improvement and even recovery from stroke, multiple sclerosis,
Alzheimers, Parkinson's, and Traumatic Brain Injury (TBI) has also been
demonstrated with hyperbaric oxygen, it's quite possible that the full
potential of HBOT can obviously save millions of heartaches and
billions--if not trillions--of dollars.

While each US state is free to administer their Medicaid program according
to however they want, each state must comply with the federal Medicaid law
for children--which specifically stipulates that a treatment like HBOT must
be paid for by state Medicaid plans--whether it is "covered by the state
plan or not".

Through this forum we as parents, caregivers, advocates, physicians,
journalists, therapists, and administrators will be able to exchange
information, tips, and advice on how to access HBOT for our children. This
is called freedom of assembly and freedom of speech as guaranteed by the
first amendment in the Bill of Rights of the Constitution of the United
States of America.

Together we can make a difference.

Separately we may never make a difference.

Hello all!

I am the Physical Therapy Crew Captain for the AVON Breast CA 3Day
Walk.  We are still looking for volunteers to help treat walkers for
the event.  You would be volunteering your time for one of the 3 days
and the cost is FREE.  Normally there is a $150 fee to volunteers
however time is getting short and so they have waived the fee for
Specialy Crew.

We would be treating walkers for all sorts of musculoskeletal
injuries - hamstrings, gastroc, soleus achilles, anterior tib and
quad overuse; providing education and stretching; insuring that they
can safely complete the event.  Your PT background alone is the
perfect preparation for doing an event like this.

The event is July 12th, 13th and 14th from Santa Clara to SF.  We
camp the first night in Bay Meadows (San Mateo), Day Two in Skyline
College (San Bruno) and end up in SF.  Hours available to work are
6:30AM to 9AM in the camp and then 1PM to 9PM in the next nights'
camp.

If you have 4-8hrs to donate your time, we'd love to have you!!!
These walkers are not always as prepared as they think they are and
your time and knowledge is invaluable!!!
