In response to your question, I don't recall little if any regarding
economics, finances, business plans etc.  Most likely since most "New
Grads" were not going to enter private practice or be in a position
which would require that type of info since we typically were
expected to start out as "new grads."  Not to mention that very few
of the PT's I've ran across have the interest or drive to put that
much work into becoming a business owner.  In my experience however,
I've been bit by the entrepraneur bug and am always looking to learn
from anyone who is willing to teach or provide info that might be
valuable.  The small business administrations (www.sba.gov) has
proven to be a valuable place also.  I've been researching this
off/on for almost two years and at some point will make the jump into
practice for myself and my wife (who's an OTR/L).  So, thank you for
the book info.  I'm planning on ordering a book via the APTA's
private practice section on starting a practice also.

Well w/ the population you mentioned, you definitely want to become a medicare/medicaid provider.  Also start calling all the HMO's in your area and ask how you can become a participating provider.  You want to build a referral basis with some doctors.  One good tip-----get a cordless phone.  If you're starting up, your employees might be yourself and/or one other person.  You don't have time to stop what you're doing to run and get the phone, so you get a cordless and carry it w/ you.  Try to get good used equipment (since it's cheaper) and try to get versatile equipment.  You don't need a lot of fancy equipment.

My name is Dan Alloway.  I am a new member to your group.  I work for
Systems4PT - a computer operating software system developed by PT's,
for PT's.  The system covers patient registration, through
documentation to zero ballance billing.

with no response, (Therapists tell me that they would not look on

In our PT program (grad USC '92) we did have to go
through the mechanics of making a business on paper
and it included budgeting, marketing, HR issues, etc.
Our professor would call attention to unrealistic
projections, etc.

In acquiring my own practice 1.5 years out of school,
there were a lot of issues that weren't covered
especially in regard to emerging managed care issues,
but it was a good exercise to learn to find and use
suitable resources.

Can't recall the text we used...it was specific to PT
practice management as I recall.

Yes I had questions about billing procedures, what are the different TOS codes from the different insurance companies:
ie I know of BCBS the 97000-97750 TOS code is "D", exam is "6"
How about the other insurance companies, UHC, Cigna; do you use a "09" for the TOS codes??.

Hello folks

I am planning to attend PTA classes starting September 2005 and I was
wondering if getting a job as a PT Aide will help me to get in a PTA
school and make connections.

In the orientation of the PTA program that I had attended, the
instructor said, most institutions are replacing Aides with PTAs
because of new regulations and constraints.

But for somebody who is planning to be a PTA, what would be the best
employment background/experience? I thought it would be to start as
an aide.

What do you guys think?

(Oh by the way I am a certified massage therapist now)

Hi,

Good luck in PTA school.
I think that it would be far more beneficial to volunteer regularly
in a Rehab Unit or in a PT Dept of a hospital than to become an
aide.  You would benefit from the exposure to a variety of Diagnosis'
The fact that you know massage means that you know one aspect of PT.
Rather than finding an aide position that utilizes something you
know, you could learn other aspects that you don't.

Definitely, do it yourself to keep overhead down.  There are some great programs (software) that make billing very easy. Examples are:  Medisoft and Medware.  There are others, gotta look around.  Initial costs are in the thousands but the programs have many dfiferent options that facilitate billing and pt scheduling (they have recall lists, appointment shceduling, etc.).  If you choose to hire a billing company, they charge anywhere from 5-8% of what they collect.

I’ll throw out an apposing view.

 

The cost of billing related to the extent of available, (ie free),  resources.

 

Assuming that available software packages are used:

In my experience the variable costs of billing, (supplies, postage, software), average ~ 0.7%.

If the start up therapist, (or her/his receptionist, or perhaps a spouse),  has enough free time to spend 15-30 minutes billing/collecting each claim …then theoretically the clinic could bill/collect for a cost less than 1% of billings.

 

Obviously as soon as the clinic needs to pay for the hours spent billing, the cost of billing increases.

As an example:

If the person who does the billing/collecting work charges $12.00 per hour.

And if total touch time for billing/collecting/posting/reconciling/re-submitting/chasing past due is 25 minutes per claim, (I think this is low).

Assuming you charge $71.00 per claim….your billing/collection personnel cost is 7.0% of billings.

This, plus your variable costs of (0.7%), identified above, totals a 7.7% cost of billings.  

 

 

Finally… the quality of the job done needs to be taken into consideration.

Average write offs seem to run in the 3.0% range.   ß Let’s assume your business is average.

 

Shop around with billing services and ask what their average write off rates are.

Ask if they will guarantee that level… or any other level.

If you find a billing service that has a current run rate of 2.5% write offs… then you’ve found another opportunity.

Your write off experience of 3.0%  minus 2.5% write offs accomplished by a billing service = a potential increase in profit of 0.5% of billings.

 

From the scenario above, (you paid a part timer $12.00 per hour to bill/collect),  where billings cost your clinic 7.7%, add the 3.0% of incremental write off to this number.

With this perspective, your in house billing/collections is really costing you 10.7% of billings.

 

Whereas a billing service that charges 6.5%, (but averages 2.5% write offs), will cost you, net, 9.0% of billings.

 

IF:

-          You pay someone $12.00/hour to bill/collect.

-          Each claim takes 25 minutes to bill/collect.

-          Your write offs are 3.0%

-          You find a billing service that charges 6.5% and averages 2.5% write offs….

 

Wouldn’t you be better off using this billing service?

-          Aren’t your real costs 9.0% with the billing service and 10.7% in house?

-          Wouldn’t your profit increase by 1.7% of billings by using the service?
