Wire and install as per photo.
The sixth band switch wire (black J5) does not need to
be attached to PCB directly: it goes via 1K resistor to ground
so it can be connected to neg. of your power supply (install 1K resistor in series).
Therefore only 5 wires are required, connected to 5-pin connector.
